These forms are generated by the district, Garfield and the PTSA.  Some forms aid the student’s well being while others contribute to school support.  Please take the time to scroll through the full listing.  Print the forms that are relevant to your family, fill them out, sign and return them to the main office at Garfield or mail to GHS 400 23rd Avenue, Seattle, 98122.
